I. Genetické minimum - chromozómové choroby a ich diagnostika

Úvod
Genetické choroby vznikajú v dôsledku mutácii na úrovni génu, alebo väčšej časti genómu, v interakcii s exogénnymi faktormi. Výskyt geneticky podmienených patologických stavov ( tab.1), alebo predispozícia k nim sa podľa viacerých štatistík odhaduje na 12-15% (1,2).
Etiologickú rôznorodosť spôsobujú rôzne typy genetických defektov, detekcia ktorých si vyžaduje špecifické laboratórne metódy (tab.1). V ostatnom čase sme zaznamenali ich významný pokrok, týkajúci sa najmä rozvoja genetických diagnostických testov, ktoré majú priamy dopad na manažment komplexnej starostlivosti o pacientov a rizikových členov rodiny. Pre včasnú diagnostiku a prevenciu genetických ochorení je dôležité vzdelávanie a transfer poznatkov do praxe každého lekára. Potrebe začlenenia najnovších poznatkov z medicínskeho výskumu a genetiky do pregraduálneho a postgraduálneho vzdelávania sú venované osobitné príspevky na blížiacej sa konferencii AMEE (Association of Medical Education in Europe). Forsíruje sa dostupnosť vedomostí s cieľom rozpoznať genotypo-fenotypové korelácie najmä vo vzťahu k liečbe a celkovej zdravotnej starostlivosti (3). Iné asociácie a spoločnosti (ESHG, OECD) odporúčajú vzdelávanie v uvedených oblastiach vrátane genomiky a proteomiky zaradiť aj do mimoprofesného vzdelávania pracovníkov v zdravotníctve ako aj pre širokú laickú verejnosť. Primeraná úroveň vzdelania zbavuje neopodstatnených obáv z genetiky, ktorá je v 21.storočí neoddeliteľnou súčasťou liečebno-preventívnej starostlivosti.
S potešením možno konštatovať, že uvedené odporúčania u nás viac rokov akceptujeme. Otázky genetického testovania, algoritmy genetickej diagnostiky, správnej laboratórnej praxe, etiky a legislatívy v odbore sú náplňou pravidelných školiacich aktivít na SZU.
Zároveň sú pre nás ambíciou zaplniť priestor v tomto periodiku na pokračovanie, ktorého obsahom bude minimum genetických poznatkov. Prvú časť venujeme chromozómovým ochoreniam a prehľadu ich diagnostických metód.
I. Chromozómové choroby
Chromozómové anomálie pozorujeme ako zmeny na chromozómoch , ktoré môžu byť :
- vrodené (konštitučné)
- získané (v priebehu života napr. malígnym procesom v bunke).
Vrodené chromozómové anomálie spôsobujú zmenou počtu, alebo štruktúry chromozómov génovú nerovnováhu a upozornia na seba závažnou poruchou morfogenézy. Pre autozómové anomálie je typický výskyt VVCH spolu s mentálnou retardáciou (obr.1). Anomálie pohlavných chromozómov sprevádzajú poruchy rastu a somatosexuálneho vývinu a sú z klinického hľadiska menej závažné. Vyhľadávanie ich zvýšeného rizika sa uskutočňuje celoplošným prenatálnym biochemický skríningom ( sAFP a betaHCG ) u tehotných žien v rozmedzí 16-18 týždňa gravidity. Vrodené a získané chromozómové anomálie potvrdíme cytogenetickým vyšetrením pri ktorom sa stanoví karyotyp. Výskyt chromozómových chýb v populácii živorodených je cca 1% (2.4).
Diagnostické známky na klinickej úrovni
Na chromozómové ochorenia u novorodenca upozorňuje prítomnosť vrodených vývinových chýb (VVCH) , ktoré sú zastúpené v ich etiológii cca 5%. Signálmi pre chromozómovú anomáliu bývajú nevysvetliteľná polystigmatizácia, tonusové poruchy, rôzne patologické parametre zistené prenatálnym ultrazvukovým vyšetrením plodu ako sú: mikrocefalia, zhrubnutá nuchálna riasa, rastová retardácia a rôzne orgánové anomálie a iné. Neskôr to býva mentálna retardácia, zaostávanie rastu, pubertálneho vývinu, nástupu menštruácie a iné. V dospelosti sa konštitučná chromozómová anomália diagnostikuje skôr výnimočne. Ich záchyt pozorujeme pri vyšetrení párov pre poruchy fertility a reprodukcie v podobe anomálií pohlavných chromozómov, zriedkavých foriem malých gonozómových mozaík a vyvážených chromozómových prestávb.
Charakteristika a rozdelenie
Predstavujú odchylky od normálnej chromozómovej výbavy somatickej bunky, ktorá obsahuje 22 párov autozómov (1-22 ) a pár pohlavných chromozómov – gonozómov (XX, alebo XY). Najčastejšou príčinou vzniku aneuploidie je nondisjukcia, alebo patologické rozdelenie chromozómov v meióze, ktoré vzniká prezygoticky. Zriedkavo sú dôsledkom postzygotickej poruchy mitotického bunkového delenia. Tieto javy sú väčšinou náhodné, aj keď sa pripúšťa, že určité rodiny môžu mať sklon k nondisjuncii (asynchrónne delenie pri predčasnom delení centroméry). Len istá malá časť chromozómových anomáli vzniká zákonite v dôsledku vyvážených anomálií (prestavieb chromozómov), ktoré sa vyskytujú v populácii ich zdravých prenášačov s frekvenciou menej ako 0,2% (4)
Rozdelenie podľa (I a II) zohľadňuje typy chromozómov a konkrétne zmeny.

A./ AUTOZÓMOVÉ
B./ GONOZÓMOVÉ
II. Podľa typu zmien v počte a štruktúre chromozómov:
a/ numerické - odchýlky od normálneho počtu diploidnej sady chromozómov somatickej bunky. Môže ísť nadpočetný chromozóm z páru ( trizómia ), chýbajúci ( monozómia ), dva nadpočetné chromozómy z páru ( tetrazómia ) , násobky sady chromozómov ( triploidie ) a iné odchylky od normálneho počtu (aneuploidie ). Z autozómových anomálií k nim patrí trizómia 21 - Downov syndróm. Ide o najčastejšiu chromozómovú anomáliu, ktorej výskyt populácii je od od 1:750-1000 živorodených. Medzi ďaľšie zvyčajne neonatálne letálne trizómie patria Patauov ( trizómia 13 ) a Edwardsov ( trizómia 18 ) syndróm.
Trizómia môže byť voľná, náhodná forma s nízkym rizikom rekurencie, alebo translokačná. Táto mohla vzniknúť ako mutácia de novo, alebo dôsledkom vyváženej translokácie jedného z rodičov. Z gonozómových anomálií má najčastejší výskyt monozómia X, alebo Turnerov ( karyotyp 45,X0 ) a Klinefelterov syndróm ( karyotyp 47,XXY), ktoré sa zisťujú v čistej, alebo mozaikovej forme. Výskyt vybraných chromozómových anomálií je v tabuľke č.2.
b/ štruktúrové - charakterizuje zmena štruktúry chromozómu vizualizovateľné ako prestavby krátkeho (p) a / alebo dlhého ramienka (q) chromozómu. Patria sem chýbanie určitého úseku chromozómu (delécia), alebo zmnoženie (duplikácia) istého úseku chromozómu. Ďalej sú to premiestnenie časti chromozómu na iný (translokácia), alebo iné tvarové zmeny chromozómov ( prstencový chromozóm, izochromozóm). Príkladom autozómovej štruktúrovej anomálie je syndróm mačacieho plaču (Cri-de-Chat sy ), ktorý spôsobuje delécia úseku krátkeho ramienka chromozómu č.5 (obr.2) Zjednodušený zápis karyotypu u chlapca s týmto syndrómom je 46,XY, 5p-. Z gonozómových anomálií sa pomerne často vyskytuje prstencový chromozóm X - karyotyp 45,X r(X), alebo izochromozóm, ktorý sa u ženy manifestuje ako Turnerov syndróm (5). Submikroskopické zmeny, kryptické translokácie nie sú možné touto metódou obsiahnúť.
c/ mozaiky: predstavujú prítomnosť viacerých bunkových klonov s rozdielnou chromozómovou výbavou. Pozorujeme ich v bunkách ako rôzne chromozómové línie, nezriedka ako minoritné, s frekvenciou menšou ako 10%. Na ich detekciu je potrebné zhodnotiť väčšie množstvo mitotických figúr. Časté sú napríklad mozaikové formy Turnerovho syndrómu, Klinefelterovho syndrómu. U týchto pacientov teda nájdeme v bunkách karyotyp s normálnym a patologickým chromozómovým nálezom. Zápis karyotypu obsahuje viacero línii a začína sa zvyčajne najpočetnejšie zastúpenou. Mozaika Turnerovho syndrómu môže mať aj takýto nezvyčajný karyotyp ( 45,XO/46,XX/46,XY ). V tomto prípade prevažuje línia s chýbajúcim X chromozóm, prítomná je aj normálna a minoritná s prítomnosťou Y chromozómu. Existujú aj iné varianty s prstencovým, alebo s izochromozómom.
Genetické diagnostické testy
Chromozómové anomálie najčastejšie diagnostikujeme laboratórnymi technikami, ktoré poskytujú kompletnú analýzu karyotypu. V ostatnom čase sa klasické metódy identifikácie chromozómov dopĺňajú o nové zobrazovacie metódy, ktorými sa odhaľujú predtým nepoznané prestavby na submikroskopickej úrovni (5,6).
Klasická metóda cytogenetického vyšetrenia je založená na analýze metafázových chromozómov získaných mitotickým delením kultivovaných lymfocytoch, ktoré sa farbia konvenčne a prúžkovacími metódami (C, G, R, Q). Detekuje numerické a štruktúrové anomálie na úrovni do 550 bandov. Vyššiu rozlišovaciu schopnosť dosiahneme metódou HRT (High resolution method), kde sa hodnotia dlhšie prometafázické chromozómy. Vyšetrenie karyotypu touto metódou vyžaduje odbornú skúsenosť a pre jej náročnosť ju nahrádzajú iné modernejšie metódy farebnej cytogenetiky (6).
Klasická cytogenetika sa rutinne využíva v diagnostike konštitučných chromozómových anomálií u pacientov s vrodenými vývinovými chybami (VVCH), mentálnou retardáciou ( MR ), poruchami rastu a pohlavného vývinu, alebo pri opakovaných spontánnych potratov. V neposlednej rade aj v prenatálnej diagnostike u plodu, ktorá sa uskutočňuje po stanovení ich rizika klinickým genetikom. Gonozómové anomálie, najmä v nizkopercentnej mozaikovej forme (Turnerov, Klinefelterov syndróm) sa môžu výnimočne diagnostikovať až pri poruchách reprodukcie v dospelom veku. Klasická metóda umožní záchyt štruktúrových anomálií a mnohonásobných chromozómových prestávb, ktoré sú typické pre syndrómy chromozómovej instability s rizikom malignity. Príkladom sú Fanconiho anémia, Ataxia teleangiektázia, Nijmegenský syndróm a iné. Manifestujú sa zvýšenou lomivosťou chromozómov. Najčastejšie sa pri týchto syndrómov zistí instabilná chromozómová translokácia 7/14, spolu s inými štruktúrovými zmenami a figúrami ( gapy, breaky, triradiály, kvadruradiály), ktoré bývajú v rôznom percentuálnom zastúpení (obr.3).
Fluorescenčná in situ hybridizácia ( FISH ) metóda používa metódy molekulovej cytogenetiky. Umožní diagnostiku chromozómových prestávb na submikroskopickej úrovni ( < 800 bandov ), ktoré nie sú vizualizovateľné klasickou technikou . Ide najmä o mikrodelečné a mikroduplikačné syndrómy a pomáha tiež odhaliť nízkopercentné mozaiky. Princípom je detekcia signálov po hybridizácii pacientovej chromozómovej DNA so špecifickými sondami (centromerickými, celochromozómovým, alebo lokusovo špecifickými) použitými podľa potreby (obr.4 ). Centromerické sondy sa používajú na zisťovanie numerických chromozómových chýb s dôkazom prítomnosti centromér, telomerických sekvencií a obsahu heterochromatínu. Celochromozómové sondy hybridizujú so svojim homológom pozdĺž celého chromozómu. Sú vhodné na diagnostiku štruktúrových anomálií chromozómov ako sú translokácie, delécie, inverzie. Lokusovo špecifické, alebo génové sondy hybridizujú do oblasti hľadaných konkrétnych sekvencii. Tento typ sond, veľkosti cca 150 kb, sa využíva k diagnostike mikrodelečných syndrómov ako sú napríklad najznámejšie z nich ( Praderov-Williho, Angelmanov, Di-Georgov, Wiliamsov-Beuerenov). Výhodou tejto metódy je hodnotenie signálov na väčšom počte chromozómov aj bunkových jadier. Použitie FISH a iných DNA metód detekujúcich sekvencie v oblasti SRY chromozómu Y umožňuje pri mozaikových formách Turnerovho syndrómu vyhľadávať pacientky s rizikom gonadoblastómu.
Kombinácia klasických a molekulovo cytogenetických metód sa využíva v onkogenetike pri diagnostike a liečbe hemoblastóz, vrátane monitorovania reziduálnej choroby a postransplantačných stavoch. Chronickú myeloickú leukémiu potvrdzuje výskyt chromozómu Philadelphia, ktorý vzniká v dôsledku translokácie medzi dlhými ramenami
Iné modifikované techniky
CGH - Komparatívna genómová hybridizácia je založená na porovnávaní dvoch fluorescenčne značených vzoriek DNA ( referenčnej a pacientovej), ktoré hybridizujú s normálnymi metafázovými chromozómami. Pacientová testovaná DNA je značená zeleným flurochrómom a referenčná vzorka ( DNA od zdravého jedinca s normálnym karyotypom ) sa označuje červeným. V pomere 1:1 sa obidve nechajú hybridizovať na preparáte s normálnymi chromozómami. Nerovnováhu v testovanej vzorke zistíme vyhodnotením pomeru intenzity zelenej a červenej fluorescencie pomocou špeciálneho softwaru. Využíva sa v nádorovej cytogenetike na skrínovanie chromozómovej nerovnováhy. Metóda však presnejšiu identifikáciu prestavby neumožňuje.
Multicolor FISH- mnohonásobná farebná fluorescentná in situ hybridizácia K detekcii zmien sa používa zmes rozdielne značených sond 5 fluorochrómami a ich vzájomnou kombináciou, čím sa docieli rozlíšenie všetkých chromozómov. Na vyhodnotenie je potrebná digitálna zobrazovacia mikroskopia s filtrami, CCD kamera a počítačový software k analýze obrazu. Na základe fluorochrómovej kompozície sa každému chromozómu určí tzv.pseudofarba..Táto metóda sa používa na identifikáciu zložitejších translokácií a prestavieb medzi viacerými chromozómami. Neumožňuje detekciu vnútrochromozómových zmien typu inverzií a malých delécií.
SKY - spektrálne karyotypovanie je variantom mnohofarebnej FISH, každému chromozómu je tiež pridelená pseudofarba. Kombinuje snímanie obrazu trojakým filtrom CCD kamerou s Fourierovou spektroskopiou a príslušným počítačovým programom.
mBAND - metóda mnohonásobného pruhovania - je založená na podobnom princípe ako FISH. Podstatou m.-pruhovania je označenie jedného chromozómu viacerými značenými sondami. V kombinácii sa tiež používa 5 fluorochrómov. Každá sonda hybridizuje s inou časťou chromozómu. Na základe hybridizačných signálov dochádza k tvorbe priečnych farebných pruhov po celej dĺžke chromozómu.
Záver: Zlatým štandardom klinickej cytogenetiky je vyšetrenie karyotypu klasickou metódou. S pribúdajúcimi dôkazmi o výskyte submikroskopických chromozómových zmien, kryptických prestávb modernými laboratórnymi technikami sa zvyšuje pravdepodobnosť odhaliť kauzálnu súvislosť s etiologicky neobjasnenou mentálnou retardáciou a polystigmatizáciou.
